Desquamative interstitial pneumonia associated with concurrent cytomegalovirus and Aspergillus pneumonia in a renal transplant recipient.

چکیده
Cytomegalovirus (CMV) and Aspergillus fumigatus are opportunistic pathogens that cause fatal infections in graft recipients mainly 1–6 months after transplantation, the period of maximal immunosuppression required for the prevention of acute graft rejection. A CMV infection has been known to increase the risk of fungal infection in an immunocompromised host, and concurrent CMV andAspergillus pneumonias have been reported in bone marrow transplant recipients [1]. Desquamative interstitial pneumonia (DIP), considered to be a non-specific pulmonary reaction to injury, is pathologically characterized by the presence of numerous macrophages within the airspace with minimal fibrosis. We report here on a patient who in his ninth post-transplant year suffered from DIP following concurrent CMV andAspergillus pneumonias, and who was treated successfully with corticosteroids combined with antiviral and antifungal agents.

An unusual pulmonary complication of cytomegalovirus infection in a renal transplant recipient
Bronchiolitis obliterans organizing pneumonia (BOOP) is a clinicopathological entity occurring in the clinical setting of interstitial pneumonia [1]. Occurrence of BOOP in the post-transplant period has been well described in lung [2] or bone marrow transplantation [3] but remains rare in renal transplantation [4]. We report here, to the best of our knowledge, the first case of BOOP secondary t...

Idiopathic pulmonary fibrosis (cryptogenic fibrosing alveolitis) is a disorder with a very poor prognosis for patients who do not respond to therapy with corticosteroids alone or in combination with immunosuppressive drugs, e.g. cyclophosphamide or azathioprine. For patients with end-stage disease, lung transplantation remains the only possibility for long-term survival. We describe a patient w...

Opportunistic infections are common in patients after renal transplantation. This report describes a case of cytomegalovirus pneumonia in a renal transplant recipient with a normal chest roentgenogram and normal arterial oxygenation. An abnormal 111In-white blood cell scan led to the discovery of a pulmonary source of his recurrent fevers.
متن کاملNocardia cyriacigeorgica bacteraemia presenting with cytomegalovirus disease and rapidly fatal pneumonia in a renal transplant patient: a case report
INTRODUCTION Nocardia cyriacigeorgica bacteraemia has been described in the setting of profound immunodeficiency in only two previous case reports. In both instances, diagnosis was rapidly facilitated by 16S rRNA gene sequencing of blood culture isolates. To the best of our knowledge, we believe that our case is the first presentation of N. cyriacigeorgica bacteraemia associated with acute cyto...
